Therapy Fees

The initial evaluation is 60 minutes in length and is $275.  

Subsequent individual sessions are 45-50 minutes in length and are $215.

During our first scheduled session, you will be sent information via text to put a credit card on file.


Insurance

I am an out-of-network provider for all insurance companies.  Please check with your insurance company to determine the parameters of your out-of-network mental health benefits. The best two questions to ask are:

1. "Do I have out of network mental health benefits?"
2. If yes, "What will I be reimbursed for an out-of-network provider?"

In out-of-network situations, I collect the full fee at the time of service. At your request, I will issue a statement to you to send to your insurance company for reimbursement.


Forms

All paperwork needs to be completed and returned before we schedule your first appointment. If you have questions you would like to address before your appointment, feel free to contact me. We can also discuss your questions when we meet face-to-face.

  2. Policies and Procedures Form (please sign page 6, indicating you have read and agree to the terms of the consent form). 

  3. Client Acknowledgement Letter (please sign)

  4. The Notice Form is for you to keep for your records

  5. The Telehealth Form (please sign in case we need to move a Telehealth session. This is very helpful in cases where you cannot travel to your appointment because of illness or otherwise but are able to meet remotely.
